The National Transitions of Care Coalition (NTOCC) has invited Tracey Moorhead, VNAA’s CEO and President, to serve on its Advisory Council to provide a home health and hospice industry perspective. The Advisory Council provides strategic recommendations and direction to NTOCC’s Board of Directors. 

NTOCC is a national nonprofit organization dedicated to raising awareness of the importance of transitions in improving health care quality, reducing medical errors and enhancing clinical outcomes.The NTOCC Board of Directors works with over 30 industry-leading professional associations, medical specialty societies, standards bodies, regulators and government organizations, which make up the Advisors Council, to guide and develop NTOCC tools and resources. NTOCC serves not only health care professionals but also policy makers, the media and consumers.

NTOCC has created white papers and statements; tools to establish safer transitions; and resources for practitioners and policy makers to improve transitions throughout the health care system. Many of these resources are available free of charge on the NTOCC Website. NTOCC also offers events and a web-based evaluation tool for institutions ready to move to the next phase of their transition of care (TOC) quality improvement efforts.
