On July 19, the Centers for Medicare and Medicaid Services (CMS) released a fact sheet on “Transitional Care Management Services.” Home healthcare and hospice agencies may find this information useful in their partnership with other healthcare providers. The fact sheet outlines:
  • Requirements for TCM services,
  • Health care professionals who may furnish TCM services,
  • TCM services settings,
  • Components included in TCM, billing TCM services, and
  • Frequently Asked Questions.
The fact sheet helps to explain Medicare reimbursement for two Current Procedural Terminology (CPT) codes that are used to report physician or qualifying non-physician practitioner (NPP) Transition Care Management (TCM) services as of January 1, 2013. TCM services may be provided to a patient following a discharge from a hospital, SNF, or CMHC stay, outpatient observation, or partial hospitalization.  If legally authorized and qualified to provide the services by the State, the following NPPs are eligible:  Certified nurse-midwives; Clinical nurse specialists; Nurse practitioners; and Physician assistants.
