CMS Outlines Electronic Template for F2F Documentation

On April 22, the Centers for Medicare and Medicaid Services (CMS) held a Special Open Door Forum to introduce the suggested eClinical template for home health. The presenters explained that the template is a data set and not intended to be mandatory for physicians. A PowerPoint presentation, which describes an overview of the project, is available here.

It is important to note this is a proposed template that would allow electronic health record vendors to create prompts to assist physicians when documenting the Home Health (HH) face-to-face encounter for Medicare purposes. CMS is currently collaborating with the Office of the National Coordinator for Health IT (ONC) and the electronic Determination of Coverage (eDoC) workgroup in developing the interoperability standards necessary for an electronic clinical template.

VNAA recognizes that there are concerns and is working with CMS to address these concerns. VNAA welcomes additional feedback from members and invites you to join our Public Policy conference call tomorrow, Wednesday, April 30 from 1:00 to 2:00 p.m. ET, during which VNAA will update members on actions to address problems with face-to-face documentation. CMS will host series of Special Open Door Forum calls to provide an opportunity for physicians, home health agencies and/or all other interested parties to provide feedback on the suggested eClinical template for possible nationwide use. .

All ODF calls are from 3:00 p.m. – 4:00 p.m. ET on the following dates:

  • Thursday, May 8
  • Thursday, June 19
  • Wednesday, July 16

VNAA will be participating on the calls and will keep members informed of the developments.
