Last Thursday, Oct. 29, 2015, CMS issued proposed discharge planning requirements for home health agencies to enhance transitions of care, patient-engagement, and care coordination across the healthcare system. CMS estimates that the proposed provisions will cost HHAs approximately $300 million to implement. Read VNAA’s summary of the rule.
