The Best Practice Work Group is composed of mission-driven home health care and hospice agency leaders who are focused on providing cutting-edge, evidence-based solutions to prevent hospitalization: 13 clinical experts from VNAA member agencies, three nurses from Home Health Quality Improvement (a QIO funded by CMS) and a nurse academic from Case Western University. The group has been tasked with developing evidence-based best practices for home health care agencies to prevent hospitalization, and since August 2012, developed approaches that will be used by agencies participating in the Engaging in Quality Learning Collaborative.
 
To date, the Best Practices Work Group has developed a set of interventions as well as educational programming on the skills needed by clinical staff. The goal of these interventions is to help agencies hardwire best practices to achieve consistent high-quality outcomes. As home health care evidence-based practices are not clearly documented in current literature, the group sought out research and evidence-based practices from other health care sectors and our members’ own expertise; these findings will add to the establishment of standards for practice in home health today.
