The Home Health Quality Improvement (HHQI) National Campaign has been helping home health agencies improve the quality of care delivered to patients since 2007. By developing free educational tools and programming and customized data reports, HHQI has helped these agencies reduce avoidable hospitalizations, improve medication management, and prevent falls and infections. In 2013, HHQI has added Cardiovascular Health to its diverse menu of quality improvement topics.

You can also download the first of two Cardiovascular Health BPIPs today for evidence-based tools, resources and research on aspirin therapy and blood pressure control and sign up for the Home Health Cardiovascular Data Registry to measure your progress.
