The Visiting Nurse Associations of America (VNAA) recently released a new quality improvement and workforce-training product, the VNAA Blueprint for Excellence. Primarily designed for use by home health organizations to promote quality in home health care practice and care transitions, it is also a valuable resource for payers, policymakers, researchers and others with a stake in improving care transitions.

Available at no cost, the VNAA Blueprint includes research- and practice-based tools and training, as well as measurement and evaluation resources to guide home health care practices. This web-based tool--available at
www.vnaablueprint.org--is easy to use, easy to access and available any time. It includes four modules--Care Initiation, Clinical Conditions, Patient Engagement and Patient Safety--covering 10 topics. The VNAA Blueprint resources are vetted by experts and represent leading expertise in the field.
