Home-based care providers have successfully engaged in pilot and small scale alternative payment models led by physician groups and health systems. Enactment of the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015 repealed the flawed Medicare sustainable growth rate (SGR) formula for physician payments and established two new payment tracks for physician practices: the alternative payment model (APM) track and the Merit-Based Incentive Payment System (MIPS) track.
