Participate in the National Palliative Care Registry

If your program provides palliative care services in patients' homes, you can enter your data in the National Palliative Care Registry. The registry serves palliative care programs across service delivery sites and is free and open to all.

Participate in the registry in order to:
  • Shape the national profile and awareness of palliative care in the home setting. 
  • Help to identify models of palliative care delivered in the home.
  • Contribute to the development of key metrics in home palliative care for future reporting.
The Center to Advance Palliative Care (CAPC) maintains the National Palliative Care Registry. The registry is the central resource for comparative data, reporting, metrics and recent research on the operational features of palliative care programs. 

A summary report on home palliative care programs will be available later this year.
