VNA Health Group Receives CMMI Community-Based Care Transitions Award

On March 13, the Centers for Medicare and Medicaid Services (CMS) announced that the Visiting Nurse Association Health Group, in conjunction with an Area Agency on Aging and other partners, received a Center for Medicare and Medicaid Innovations grant for a Community-based Care Transitions Program (CCTP) in three counties in New Jersey. To read the full press release click here.

The Community-based Care Transitions Program (CCTP), created by Section 3026 of the Affordable Care Act, tests models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.

For CCTP grants, CMS gives preference to Administration on Aging (AoA) grantees that provide care transition interventions in conjunction with multiple hospitals and practitioners and/or entities that provide services to medically-underserved populations, small communities, and rural areas.
