Partners HealthCare at Home  is part of Partners HealthCare, founded by Massachusetts General Hospital and Brigham and Women’s Hospital, serving Eastern Massachusetts. Partners HealthCare at Home has the knowledge and expertise of an entire health system – including the affiliated rehabilitation hospitals and outpatient centers of the Spaulding Rehabilitation Network – available to patients and caregivers. Together, Partners HealthCare at Home and the Spaulding Rehabilitation Network constitute the non-acute care services division of Partners HealthCare. Winona Health Home Care and Hospice is part of Winona Health, a nonprofit integrated healthcare provider serving patients in the Winona area and Southeast Minnesota. The Walpole Area Visiting Nurse Association (WAVNA) is an independent, non-profit, community-based home care agency, providing high quality home health care and resource information to people of all ages. WAVNA strives to respond to patient and community needs, to promote healthy lifestyles and facilitate independence enabling people to remain in their own homes, enhancing quality of life and reducing hospitalizations. WAVNA works with all major Boston hospitals and local community hospital networks to provide the skills and technology necessary for seamless transitions to home.
